Data Collection and Assessment Methods
During phase one, I plan to implement lessons that focus on self-efficacy, self-awareness and problem solving. Throughout this phase, I will collect feedback in three ways: a teacher reflection journal, behavior observation forms, and student feedback slips.
In my teacher reflection journal, I will take notes about observations throughout the day, focusing on my informal conversations and interactions with students. The notes will include interventions needed in helping students solve problems, feelings and concerns students express to be in our conversations, and my general feelings and reflections on occurrences and progress made from each day.
In my behavior observation forms, I will record daily occurrences individually for each student. On this form I will separate behavior out by positive and negative behaviors and take notes about specific situations in which these behaviors are exemplified. While I plan to focus on the positive behaviors, I still find it important to document and identify negative behaviors, so I can address the behaviors in a positive way. An example of the form looks like this:

The student reflection slips are my formal way of assessing the students understanding of self-efficacy, self-awareness and problem solving. For this, I will give the students exit slips and have them respond to a question in writing and/or draw a picture. Sample questions include:

2. How did you help a friend today?
a. This is focused on self-awareness and problem solving
3. How did you solve a problem with yourself today?
a. This is focused on self-awareness, self-efficacy, and problem solving. This is in reference to them solving
their own problem such as tying their shoes, finding a sharpened pencil etc.
4. How was I good at solving problems today?
a. This is focused on self-awareness, self-efficacy and problem solving. This is also in reference to them
reflecting on their own independent problem solving.
